NHS prescriptions
Nominate us for your NHS prescriptions
Ask Lower House Pharmacy to be your nominated pharmacy. Fill in the form and the pharmacy team sets it up on the NHS system.
Before you start
- Nominating means your GP surgery sends your NHS prescriptions straight to the pharmacy you choose.
- It is free, and you do not have to nominate a pharmacy.
- You can choose any pharmacy, and change or remove your nomination at any time.
- You can also nominate us yourself in the NHS App, or ask at the counter.
- If you are asking for someone else, such as your child, please call us on 0151 546 2294 instead.
Thank you, we have your request
The pharmacy team will set your nomination on the NHS system. Your GP surgery then sends your NHS prescriptions to us.
You can change your nomination at any time, in the NHS App, at any pharmacy or with your GP surgery.
If you have a question, call us on 0151 546 2294.
